AMENDMENTS AND CORRECTIONS

In the approximations for a product, and the subsequent calculations, a factor of ½ was omitted. The approximations should be on p. 251, on p. 266, and on p. 267. Corrected values for D in Table 8, p. 268 are available from the author. The approximations are now even better in the situations of interest.

The value for A when {pi} = 1 for process (c3) in Table 10, p. 273 is 0·839. On p. 267 the two occurrences of ‘’ should be removed. The year for the Schuster et al. paper is 1978. On p. 276 the Hedayat & Federer reference should read ‘nonexistence’ and not ‘existence’.
